PHILADELPHIA – September 14, 2017 – Turn-Key Health (TKH), an advanced illness population health management (PHM) company serving payers and their members, announces the release of the first part of its three-part white paper, “Effectively Addressing the Impact of Advanced Illnesses.” This series expands the understanding and analysis of the 12-18 month time period during the last stages of serious illness, and provides an introduction to new, innovative models of care that successfully address these challenges for 1) health payers, 2) providers and the U.S. healthcare system, and 3) consumers.

Part One, “Recalibrating Population Health Management to Include Community-Based Palliative Care,” examines specific issues for health payers, and how Turn-Key’s innovative, proprietary model Palliative Illness Management™ (PIM™) augments traditional PHM to deliver improved quality of life for members and caregivers facing the many burdens associated with advanced illness.

“By integrating PIM™ within existing broader PHM frameworks, payers can identify and effectively address significant quality and cost improvement opportunities in member populations grappling with advanced illness,” says Greer Myers, president, Turn-Key Health. “By identifying members accurately, engaging appropriately through community-based palliative care clinicians, and standardizing approach through technology, we are able to dramatically improve the consumer experience. Avoidance of non-beneficial treatment and unnecessary episodes of acute care at end-of-life benefits both the consumer as well as the payer.”

He adds that end-of-life care today must focus less on the disease process and more on the consumer’s goals of care. Effectively improving quality within this population requires a comprehensive and specialized approach not inherent within most PHM programs.

Myers concludes, “We are taking a 360-degree look at end-of-life care to assess the issues impacting all stakeholders – payers, providers, and consumers. While there are varied perspectives, there is a consensus that we need to do more to improve quality of life for patients while reducing economic burdens for all.”
