Person-Centered Strategies for Palliative Care Reduce Costs, Improve Quality of Life for Members with a Serious or Advanced Illness
PHILADELPHIA – July 12, 2018 – Turn-Key Health (TKH), an affiliate of Enclara Healthcare, serving health plans, provider organizations and their members who are experiencing a serious or advanced illness, announces that John Halsey, vice president, Payer Development, Turn-Key Health will present, “Develop a Person-Centered Strategy for Palliative Care and Advanced Illness to Reduce Costs and Increase Quality of Life,” on Tuesday, 10:45 a.m., July 17, during the 2018 GHCC’s National Care Coordination Summit for Medicaid, Medicare and Duals, Alexandria, Virginia, July 16-17, 2018.
“Attendees will get meaningful guidance on the changes, initiatives and activities that are most effective in establishing a person-centric approach,” says Halsey. “With a focus on increasing patient satisfaction, care quality and cost effectiveness, the presentation will identify the key facets of payer-provider collaboration for establishing an effective care plan. The discussion will also include the role of palliative care teams to provide support to members and caregivers for everyday living outside of the acute care setting.”
This session will also address the importance of a systemized, end-to-end approach to palliative care. Halsey will offer documentation of the value of a specialized model that provides structured support and uniform, standardized clinical programs for members with a serious or advanced illness.
“With physicians at the helm of patient care and leading the Patient-Centered Medical Home, a structured model positions its palliative care teams to extend personalized services to members beyond the hospital environment,” says Halsey. “Field-based clinician resources expand the reach of a high-touch medical practice and provide relief to the time-pressured physicians. The goal is to coordinate the many day-to-day needs of the member and caregiver, freeing up physicians to provide quality medical care that improves member outcomes.”
Halsey reaffirms, “This level of support also increases the frequency of interaction between physicians and members, while enhancing long-standing physician relationships with families.
